Responsible for providing in-home physical therapy to patients. Responsibilities included evaluation of patients and development of a plan of treatment by performing diagnostic and prognostic tests as appropriate. Provide patient care in accordance with the plan of treatment. Observation, documentation and reporting patient’s response to treatment and changes in patient’s condition. Continual monitoring of a patient’s status and changing the plan of care when appropriate. Participation with other health team personnel in staff meetings, policy formation, planning care conferences, and discharge planning for the purpose of planning and evaluating patient care in individual cases, and in-service training for staff members. Education of patients and families with regard to in-home physical therapy. Instruction of other health care personnel in physical therapy treatments when appropriate. Supervision of non-licensed staff members [nursing assistants].
